Answer: There is free fluid around the caudal tip of the liver

The caudal tip of the liver is the most sensitive location to pick up free fluid in the right abdomen

The anechoic space seen medial to the liver at the end of the clip is the IVC.

Answer: There is free fluid seen filling spaces between stool filled loops of bowel

Free fluid fills potential spaces within the abdomen. As such it is generally recognized by anechoic spaces with irregular angulated borders. Anechoic spaces with regular borders forming geometric shapes are contained fluid structures until proven otherwise.

Answer: Free fluid is seen in Morrison’s pouch

This is clearly an image in the abdomen which shows the liver-kidney interface (Morrison’s pouch) as well as the spine in the far field. The spine and kidney are excellent internal landmarks to help recognize that we are capturing a view of the posterior abdomen.

Answer: Subdiaphragmatic free fluid

The image shows the inferior aspect of the chest on the left and the abdomen on the right.

The thin curved line in the middle of the image that follows the contour of the spleen and extends from the chest wall to the far field of the image is the diaphragm

The anechoic space just to the left of the diaphragm is a pleural effusion, the small anechoic sliver between the diaphragm and the spleen is ascites.

Answer: This is a contained fluid space

Note that the borders of the anechoic space are neither irregular nor anechoic. It takes on a distinct geometric form. In addition it seems to disappear at the level of the kidney. The kidney is retroperitoneal so represents a very dependent portion of the abdomen where free fluid would be expected to settle preferentially.
